The following is a list of tasks that MUST be completed prior to beginning your study at the UQ School of Pharmacy:

  1. Enrolment (see How to Enrol)
     
  2. Credit transfer form if applicable (see Credit Transfers)

The following is a list of tasks that MUST be completed prior to attending a placement:
  1. Apply for a Blue Card (see Obtaining a Blue Card)
      
  2. Complete QLD Government Workplace Health & Safety -  Safety Sense Certificates
     
    • - You should complete 2 components online: General & Community Services.
       
    • - Once completed, you should print the certificates and hand into the School of Pharmacy office with your Blue Card application. 

Your completed Blue Card Application and Safety Sense Certificates are due at the School of Pharmacy no later than Thursday 5 April 2012.
 
  1. View the confidentiality form (located on the School of Pharmacy Portal). Do not complete until instructed.
     
  2. See Queensland Health Essential Placement Requirements and either gather your Hepatitis B immunisation records together, organise a serology blood test to show your Hep B status, and/or commence a course of Hepatitis B vaccinations. Originals of your Hepatitis B immunisations/vaccinations/status must be provided with your completed Orientation Checklist to the School of Pharmacy office.
Due Dates
  • Incoming accelerated students, your completed Qld Health Orientation Checklist and Hepatitis B evidence is due at the School of Pharmacy office no later than Friday 1 June 2012.
     
  • Incoming 1st years, your completed Qld Health Orientation Checklist and Hepatitis B evidence is due at the School of Pharmacy office no later than Friday 26 October 2012
