- Energy intake and energy expenditure: how do we measure these accurately to determine energy balance in health and disease? Pitfalls in nutrition research with the example of motor neuron disease.
Energy intake refers to energy consumed through food and beverages. It is assessed by self-report. A variety of instruments are available to collect data to determine “usual” intake. But do they really capture accurate usual intake? Energy requirements include four components: basal metabolic rate, physical activity, thermogenesis, and diet induced thermogenesis. Energy requirements can either be estimated or measured. Both methodologies involve degrees of assumptions bringing biases into calculations.
Motor neuron disease (MND) is a group of fifty neurological conditions, characterised by the rapid degeneration of motor neurons. Prognosis of death is 2 to 5 years from diagnosis, and the prevalence worldwide is reported as 1 to 11 in 100 000. Weight loss is ubiquitous in MND and predicts rapid decline. Estimating energy requirement accurately, and assisting patients in meeting requirements is paramount in extending lifespan and quality of life.
