Your application to live at Duchesne College is just a matter of following a few simple steps:

• Download and complete the Application Form 2005 along with the Applicant's Interview Form 2005 below. You should then mail the forms to Duchesne College and phone to arrange an appointment to see the Principal for an interview.
  
• Download the Applicant's Referee Form 2005 (two copies). Forward one copy to each of your referees. These forms should be completed by your referees and returned by them to Duchesne College.
  
• We recommend you also download and read the accompanying form regarding General Information for 2005.

If you would like confirmation that your application has been received, please send a stamped self-addressed envelope.
