Lost or stolen ID cards

In the event of a lost or stolen identification card, immediately notify both the UQ Library and Security to revoke access.
Then, take a signed Requisition for Internal Services (RISQ) form stating staff name, employee number and authorised payment to your nearest Unicard outlet.

Unicard is located at:

• St Lucia campus, Duhig Tower Level 2, Social Sciences and Humanities Library (see map),
• UQ Ipswich campus Student Centre (see map); and
• UQ Gatton Student Centre (see map)

Unicard staff advise Security staff daily via email of any replacement cards issued to enable updating of the access control database. When you obtain a replacement card, a new version of the barcode is generated. Take your new card to the Loans Desk at any branch library to get your record updated to reflect the changes contained in the card.
