Fraser Coast Palliative Care Project

The Fraser Coast Palliative Care Project is an initiative to evaluate existing models of care for the provision of palliative care services in the Fraser Coast region on the central coast of Queensland.  It has been designed with a view to developing a consistent approach to care planning and service delivery for those choosing to stay at home to receive palliative care.

The purpose of this project is to develop and trial models of service that enhance care planning for all Fraser Coast people diagnosed with a life limiting illness.  It is attempting to achieve this through the:
  • Review of existing models of palliative care service provision and development of a consistent approach to care planning and service delivery for people choosing to stay in the community
  • Development of formal linkages between palliative care service providers on the Fraser Coast
  • Review of existing discharge planning processes and development of an integrated and coordinated approach to transition between settings.

In addition to these and other key personnel from Blue Care services on the Fraser Coast, the project is supported by representation by health professionals and palliative care service providers within the Fraser Coast district including the Fraser Coast Palliative Care Service, the Wide Bay Division of General Practice, public and private hospitals, community health services, indigenous health services, aged care facilities and community pharmacies.
 
Individuals and community groups with interest in issues at the end of life are represented by consumers of palliative care (past or present), indigenous groups and other community groups.
